A car insurance policy doesn’t cover a driver’s medical expenses. For this reason, a motorist needs to get a car medical form from his/her insurer. These forms are known as “drivers medical forms” or “DMFs”. The form must be completed by an authorised medical practitioner.
If the motorist has a history of heart disease or high blood pressure, he/she should get a second DMF from the doctor. The second form should be signed by two doctors. After that, the insurance company will issue a letter saying that the driver is eligible for the coverage.
Once the driver has got this letter, he/she can proceed to the medical centre where he/she is required to pay for the cost of a medical test. The total cost is usually D4 Medical between £61 and £65 plus VAT (currently £65.36). You will need to provide some details before getting a health check.
In this case, you are asked to provide your National Insurance number, which is used to calculate your premium. The next question is your date of birth. You may be asked if you are male or female.
The gender and occupation fields are optional. You are also asked to fill in your marital status. There is another line that you need to write your phone number. In addition, you may be asked for your address.
